The Practice is registered with the Information Commissioner’s Office and complies with the EU General Data Protection Regulation 2018, the UK Data Protection Act 2018 and the Access to Medical Records Act 1988.
There is no charge for this service. However, if you ask for access more than once in a 12 month period then you may be liable for a £50 fee.
For access to results, Immunisation records and medical records you will need to come into the practice with Photo ID and Proof of address dated within the last three months and fill out the appropriate subject access request form.
